This Privacy Policy explains how Saema Tahir, M.D. P.C. (“Dr. Tahir,” “we,” “us,” or “our”) collects, uses, and protects information you provide through this website. It applies only to information collected via this website. It does not cover protected health information (PHI) created or maintained in the course of clinical care, which is governed separately by the Notice of Privacy Practices you receive at your visit and by the Health Insurance Portability and Accountability Act (HIPAA).

2. The Website Is Not a Patient Portal

This website is an information and outreach resource. It is not a patient portal, telehealth platform, or HIPAA-secured communication channel. Please do not submit detailed medical history, test results, prescription requests, or any urgent or emergency-related communication through this website. PHI exchanged in the office or on a HIPAA-compliant channel remains protected under HIPAA and the practice’s Notice of Privacy Practices, which is provided separately.

If you have a medical emergency, call 911 or go to the nearest emergency room.

3. Information We Collect

We collect only the information you provide voluntarily through forms on this website. Specifically, the appointment request form and any general contact form may collect:

  • Your name
  • Email address
  • Phone number
  • Whether you are an existing patient
  • A short description of what you would like help with
  • Any other information you choose to include in a free-text field

We do not require visitors to create an account to use the website, and we do not request Social Security numbers, financial information, or insurance ID numbers through this site.

4. How We Use Your Information

We use form submissions solely to contact you about scheduling, to follow up on your inquiry, and to maintain a record of your request. We do not use your information for marketing, advertising, profiling, or sale. We do not share, rent, or sell your information to third parties.

5. Cookies and Tracking

This website uses a small number of standard WordPress cookies that are required for the site to function (for example, to support form submission and site administration). These cookies do not track you across other websites.

This website does not currently run third-party advertising trackers, social media tracking pixels, or behavioral profiling tools. If we add web analytics or similar tools in the future, we will update this Privacy Policy accordingly. We do not respond to “Do Not Track” browser signals because there is no consensus standard for them.

6. Forms Software and Service Providers

Form submissions are processed by Gravity Forms, a WordPress plugin running on this website’s hosting environment. Submission contents are stored in the website’s database and emailed to the practice. The hosting provider and email transport service may have incidental access to submission data only as necessary to deliver the message. We require service providers to handle data confidentially and to use it only for the purposes for which we engaged them.

7. Data Retention

Form submissions are retained while they are useful for follow-up and recordkeeping. We periodically review and delete entries that are no longer needed. You may ask us to delete a submission at any time (see Section 9).

8. Security

The website is served over HTTPS. We take reasonable administrative, technical, and physical safeguards to protect information collected through this website. No method of internet transmission or electronic storage is 100% secure, and we cannot guarantee absolute security.

9. Your Rights

Depending on where you live, you may have the right to:

  • Access the personal information we hold about you that was collected through this website
  • Correct inaccurate information
  • Request deletion of your information
  • Opt out of any uses of your information you have not already opted out of (we do not sell your information)

These rights apply, where applicable, under the New York SHIELD Act, the California Consumer Privacy Act (“CCPA”) as amended by the California Privacy Rights Act, and the EU/UK General Data Protection Regulation (“GDPR”). HIPAA-protected information held by the practice is governed by the practice’s Notice of Privacy Practices, not this Privacy Policy.

10. Children’s Privacy

Dr. Tahir treats pediatric patients, and a parent or legal guardian may submit an appointment request on behalf of a minor child. We do not knowingly solicit personal information directly from children under 13 through this website. If you are a parent or guardian and believe your child has submitted information through this website, please contact us so we can delete it.

11. Third-Party Links

This website may link to third-party websites — for example, news outlets that have featured Dr. Tahir’s commentary. Those third parties have their own privacy policies and practices, and we are not responsible for them.

12. Changes to This Policy

We may update this Privacy Policy from time to time. The “Last updated” date at the top of this page reflects the most recent revision. Material changes will be highlighted on the website.

Insurance and Payment Policy:

Payment is due at the time of service. Tahir, M.D. does not participate in insurance plans except for Medicare. Lab work, Radiographic studies, and Medications can be billed to your insurance.
